Can you say just where during the checkout this happened?
This happened last month at least once.  I think it was when I used mcpMenu to
turn on the _imager_ flat-field lamps and it just "went away" afterwards
without rebooting the MCP (which is what I would try first).  I was suspicious
it was a mcpMenu bug mis-setting some bits in the MCP, but didn't do anything
for lack of direct evidence.
